How can I improve this shot? The noise is high and it's not sharp enough in my opinion. Shot at ISO 800 :(

I think you pretty much nailed what you thought was wrong with the photo. How come such a high ISO and it still seems a bit dark.

Looks like alot of cropping, high iso and exposure is off. It is a tough one to get perfect with the light / bright background and the dark haired hairy guy.
At higher iso's you really have to limit the cropping.

Thanks guys. It's shot at 200mm and cropped to get this composition.
The subject is too dark but if I try to correct it, the noise comes up again.
He's one of my neighbors. I'll just wait for him to go out again
I love the composition of this though so definitely get him to come out again.

Thanks guys. I like the composition and the contrast also. But it's ruined with the noise.
Have you tried it in B/W where the noise/grain can be more acceptable? Whats causing the strong background shadow?
