Welcome first off. I'm not so sure what your asking for has been invented yet. The go-pro line does a pretty decent job and pops in your pocket. Most DSLR's shoot decent video now but don't pop in your pocket and autofocus isn't there yet IMHO. As for point and shoots I can recommend the Canon powershot line as I've used it before and have yielded good results. Some higher end phones are delivering pretty good stuff. I'd take a look at the go-pro hero 3 as it will shoot good video and save stills at the same time. It also doesn't cost an arm and leg either. Hope this helps.
