Someone has asked to use one of my photo's and I want to limit their usage to single use. Is there a form or document that I can use to do that?

You can just write one up Russ. Call it a 'Commercial Copyright and Usage Agreement' and make whatever stipulations you want and get the user to sign it.

You'd want to include that the copyright remains your property and that the person must not re-sell the photo.

You word it to the effect that you are licencing the person to a single use for such and such a purpose ... and then ... provide ONLY the appropriate size they need. If it's an A4 (or US Letter size) brochure then obviously that's the biggest you need to provide.

At least that's how it works here.

And, when you've got a rough draft that you're happy with, shake a few hundred dollars out of your piggy-bank, and contact a lawyer who specializes in this area. Show him the draft, tell him what you want to do, and get a professional opinion on whether what you've said does, in a legal sense, say provide the protection you want.

Yup TI is correct - that would be a coupla hundred well spent dollars. The sample I posted is just that, a sample and legally it MIGHT be worth what you paid for it. It was meant just to give an idea of what you might want in there.
